(B) The department of job and family services shall prescribe an application for a foster caregiver to use under division (A) of this section.
The application shall not require that the foster caregiver provide any information the foster caregiver already provided the department, or undergo an inspection the foster caregiver already underwent, to obtain a foster home certificate under section 5103.03 of the Revised Code.
